Please bring the following items to your doctor appointment: (Plan
at least one week to obtain these)
- All related FILMS AND TYPED REPORTS (X-Rays, MRI’s, CT Scans,
etc.) - All related Test Results (EMGS, Labs, etc.)
- If you have an HMO or insurance which requires a referral,
you must have the Authorization in hand to be seen or previously
faxed to office - If auto accident /work injury (comp) Liability case, you must
have a letter of authorization and claim number in hand to be
seen
